Admission Documentation

Upon admitting new residents in our residential facility, our personnel will provide the resident and their parent or guardian, custodian, or designated representative with the information listed below:

• The resident rights explain the certain rights and protections that the resident shall exercise and enjoy while on the premises.
• A detailed explanation of our payment policy the resident must pay while admitted to our facility
• A personal copy of our refund policy and procedure
• Contact information and address of:
  • Bureau of Residential Facilities Licensing (BRRL)
  • The Department’s Division of Behavioral Health Services
  • The Arizona Department of Economic Security Office of Adult Protective Services
  • The local and regional behavioral health authority
  • The human rights advocate from the regional behavioral health authority, if the resident is an individual who has a serious mental illness
• A personal copy of our agency’s grievance policy and procedure
• A detailed description of our house dress code

The residents and their parent or guardian, custodian, or designated representative shall sign an acknowledgment affirming they have been provided with the above-mentioned information. The said acknowledgment will be kept along with the residents’ files.
