Upon admitting new residents in our residential facility, our personnel will provide the resident and their parent or guardian, custodian, or designated representative with the information listed below:
- The resident rights explain the certain rights and protections that the resident shall exercise and enjoy while on the premises.
- A detailed explanation of our payment policy the resident must pay while admitted to our facility
- A personal copy of our refund policy and procedure
- Contact information and address of:
- Bureau of Residential Facilities Licensing (BRRL)
- The Department’s Division of Behavioral Health Services
- The Arizona Department of Economic Security Office of Adult Protective Services
- The local and regional behavioral health authority
- The human rights advocate from the regional behavioral health authority, if the resident is an individual who has a serious mental illness
- A personal copy of our agency’s grievance policy and procedure
- A detailed description of our house dress code
The residents and their parent or guardian, custodian, or designated representative shall sign an acknowledgment affirming they have been provided with the above-mentioned information. The said acknowledgment will be kept along with the residents’ files.
