New Oasis Group LLC will discharge a resident under the following circumstances:
- The resident has fulfilled the treatment goals as indicated in their treatment plan; or
- The resident requires a higher level of care for their behavioral problem that we are not equipped to deliver
We will prepare documentation detailing the specific needs of the resident after their discharge, as well as information about the level of care the resident may need after discharge as assessed by our contracted Clinical Advisor. The discharging plan will be completed and handed to their legal guardian/authorized representative before discharging the resident. Additionally, their medical records will be readily available within 48 hours after the process.
We will ensure that once the resident is discharged, they are already referred to the appropriate treatment or ancillary services the resident will need after leaving our facility.
