Please be guided by our transfer process.
New Oasis Group LLC will ensure close communication with the personnel from the receiving healthcare institution to coordinate the transport and services provided to the resident during the transferring process. We also guarantee to follow the state policies and procedures upon the transfer of a resident, such as:
- Delivering the residents’ medical records, which includes the date and time of the transport and the mode of transportation to the healthcare institution where the resident will be transferred to
- Evaluating the resident’s condition before and after the transport
- Explaining to the resident and their legal guardian/authorized representative the risks and benefits of the transport
- Personnel from our facility will accompany the resident during the transportation when deemed necessary.
The above-mentioned conditions will be meticulously followed except during emergencies.
